Please be advised that the process of becoming a volunteer
will include the following:
- Completion of an application
and interview with the Director of Resident Services

Completion of the following
paperwork:
- Volunteer Code of Conduct
- Volunteer Acknowledgement
(Confidentiality Agreement)
- Copy of Driver’s License or
Photo ID
- Two Completed Reference Forms
(Non-family members)
- Photo Release Form
Parental Consent (If
volunteer is under 18 years of age)
- Authorization for a
Background Check and Proof of a Clear
- Background Check
The following are also requirements:

PPD Test (Tuberculosis) *To
be completed annually.
- Proof of COVID-19 Vaccination
